Case snapshot

This case follows a 40-year-old patient through individualized extended deep plane facial rejuvenation, with a six-month follow-up. The updated plan combines lower-face and neck lifting with selective forehead, temple, midface, periocular, volume-restoration and perioral procedures. The goal was refinement and structural support while preserving facial identity.

Patient concerns

The concerns were early lower-face laxity and reduced jawline definition, neck laxity, midface descent and reduced cheek support, upper- and lower-eyelid aging, forehead and temporal descent, selective soft-tissue volume loss and downturned mouth corners.

Clinical assessment

Extended deep plane face and neck lifting provided structural support for the lower face, jawline and neck. Forehead, temple and midface lifting supported the upper and central thirds. Upper and lower blepharoplasty, micro/nano fat grafting and corner-of-mouth lifting refined periocular, volume and perioral balance.

Surgeon's note

Dr. Umut Zereyak's updated planning note emphasizes restoring support before aging becomes more advanced while keeping the patient's identity intact. Extended deep plane lifting provided the structural foundation, and the additional procedures were selected to create balance across the whole face.

Six-month follow-up

The six-month assessment considered jawline definition, neck contour and the facial-neck transition, midface support, both eyelids, forehead and temporal balance, facial volume distribution and perioral balance. This is an early-to-intermediate result, with ongoing scar and soft-tissue maturation.
